Why hip pain is complex

The hip is where everything connects.

The hip is one of the most load-bearing joints in the body and one of the most commonly misdiagnosed. Pain in the outer hip is often labelled bursitis when it is actually gluteal tendinopathy. Deep groin pain gets attributed to muscle strain when it may be labral involvement or hip impingement. The wrong diagnosis leads to the wrong treatment, which is why many hip pain patients cycle through interventions for months without improvement.

Every hip condition has a different answer.

The wrong diagnosis leads to the wrong treatment. Accurate assessment is the starting point for everything else. These are the conditions we see most often and know best.

Impingement

Hip Impingement (FAI)

Femoroacetabular impingement occurs when the hip joint has abnormal contact between the ball and socket. It produces deep groin pain, particularly with hip flexion, crossing legs and prolonged sitting. Common in active people aged 20 to 50. Arthritis Australia recognises physiotherapy as a first-line treatment for FAI, often producing outcomes equivalent to surgery when rehabilitation is thorough.

Tendinopathy

Gluteal Tendinopathy

The most common cause of lateral hip pain, affecting runners, cyclists and desk workers. Often misdiagnosed as bursitis. The gluteal tendons degenerate where they attach to the greater trochanter, producing pain when compressing the tendon, such as crossing the legs, lying on the affected side, or climbing stairs. Responds well to progressive loading programmes when managed correctly.

Bursitis

Hip Bursitis (Greater Trochanteric)

Inflammation of the bursa over the lateral hip. Often presents alongside gluteal tendinopathy. True bursitis in isolation is less common than previously thought. Pain on the outer hip, worse with lying on the side and climbing stairs. Treatment focuses on load management, offloading compressive forces on the bursa, and progressive gluteal strengthening.

Labral tear

Hip Labral Tears

The labrum is a ring of fibrocartilage around the hip socket that provides depth and stability. Tears produce groin pain, clicking or catching sensations, and reduced range of motion. Often associated with FAI. Many labral tears are managed effectively with physiotherapy focusing on hip joint centration, strength and movement retraining, without surgical intervention.

Arthritis

Hip Osteoarthritis

Gradual degeneration of hip joint cartilage producing morning stiffness, aching with activity and progressive loss of movement. The evidence for physiotherapy in hip OA is strong, with Cochrane reviews supporting exercise as the most effective conservative treatment for pain reduction and functional improvement. We treat hip OA at every stage, including pre and post hip replacement.

Sport & cycling

Sports Hip Injuries

Hip flexor strains, adductor injuries (groin strains), piriformis syndrome, hip stress fractures in runners, saddle-related hip pain in cyclists, and osteitis pubis in footballers. We work with Ciclo Melbourne on cycling-specific hip injuries where bike position is a contributing factor. Sports hip injuries require accurate diagnosis and sport-specific rehabilitation protocols.

Phase one

Accurate diagnosis

Forty-five minutes is enough time to actually understand your hip. We take a thorough history, perform movement and provocation testing, assess muscle strength and flexibility, and identify all contributing factors including training load, movement patterns and lifestyle.

Phase two

Targeted treatment

Hands-on treatment matched to your specific diagnosis. Not a generic hip protocol. Manual therapy to restore joint mobility and reduce pain. Dry needling for trigger point release where indicated. Exercise prescription that loads the right structures in the right sequence.

Managing hip pain between sessions.

What you do between appointments matters as much as what happens during them. These are the principles that consistently make the biggest difference.

  • 01
    Avoid compressive positions
    For lateral hip pain, crossing your legs, sitting with hip adducted, and lying on the painful side all compress the gluteal tendons and bursa against the greater trochanter. Minimising these positions reduces flare-ups significantly.
  • 02
    Keep moving, but manage load
    Complete rest is rarely the answer for hip conditions. The goal is maintaining activity within pain tolerance while reducing high-load activities temporarily. Walking is generally beneficial. Running may need modification.
  • 03
    Heat before activity, not ice
    For most chronic hip conditions including gluteal tendinopathy and hip OA, heat before activity improves tissue extensibility and reduces stiffness. Ice is more appropriate for acute injuries in the first 24 to 48 hours.
  • 04
    Strengthen, not just stretch
    For tendinopathy, stretching the gluteal tendons can worsen symptoms by increasing compressive load. Progressive strengthening, particularly of the gluteus medius and maximus, is the evidence-based approach for most lateral hip conditions.
  • 05
    Check your bike fit
    For cyclists, saddle height, fore-aft position and cleat alignment all influence hip mechanics. A saddle that is too low increases hip flexion at the top of the pedal stroke and compresses the hip. We work with Ciclo Melbourne on complex cycling hip cases.

How long does hip pain take to heal with physiotherapy?

Recovery depends on the specific diagnosis. Acute hip flexor strains and bursitis flares typically improve within 4 to 8 weeks. Hip impingement and labral tears require 8 to 16 weeks of targeted rehabilitation. Gluteal tendinopathy responds to progressive loading over 8 to 12 weeks but can recur without maintaining the programme. Hip OA requires ongoing management. We will give you a realistic, specific timeline at your first assessment.

Can physiotherapy help me avoid hip surgery?

In many cases, yes. Research supports physiotherapy as an effective first-line treatment for hip impingement (FAI), often producing comparable outcomes to surgical intervention when rehabilitation is thorough. For hip OA, Cochrane systematic reviews consistently show exercise-based physiotherapy reduces pain and improves function. We work with orthopaedic surgeons when surgery is genuinely the best option, but exhaust conservative management first.

What is the difference between hip bursitis and gluteal tendinopathy?

They produce very similar symptoms, particularly outer hip pain that worsens with lying on the side and crossing the legs. The key difference is the structure involved: bursitis is inflammation of the bursa fluid sac, while gluteal tendinopathy is degeneration of the gluteal tendons. They often occur together. The distinction matters because treatment differs slightly. Gluteal tendinopathy requires progressive loading while avoiding compressive positions. True isolated bursitis is less common than once thought. Accurate assessment at first appointment will differentiate them.

I have hip impingement. Should I stop exercising?

Not necessarily. The goal with FAI is identifying which movements provoke symptoms and modifying your training appropriately, not stopping exercise entirely. High hip flexion movements, deep squats and activities that recreate the catching or pinching sensation may need temporary modification. Walking, swimming and cycling with a higher saddle are usually well-tolerated. We will give you specific guidance for your sport or activity at your first appointment.

Do I need an MRI before coming to see you?

No. Imaging is useful for ruling out serious pathology and for surgical planning, but most hip conditions can be accurately diagnosed through clinical assessment. An MRI finding of labral tear or FAI does not automatically mean surgery or that those findings are causing your pain. If you have existing imaging, bring it and we will review it as part of your assessment. If we believe imaging would change your management, we will discuss referring you for it.

Do I need a referral to see a hip physio?

My hip pain is also affecting my lower back and knee. Can you treat all of it?

Yes. Hip dysfunction commonly produces compensatory movement patterns that stress the lower back, SIJ and knee. We treat the full picture, not just the loudest symptom. Often resolving the primary hip issue reduces the secondary back and knee complaints as well. If you have significant back pain, our lower back pain guide is a useful companion read.
