Most neck pain is mechanical — meaning it's driven by how you move, how you sit, and what you do every day. That's actually good news, because mechanical problems respond extremely well to physiotherapy when it's done properly.

At Evolutio we've treated every variation of neck pain since 2013 — from the "I can't turn my head this morning" acute strain to the "this has been there for three years" chronic presentation. Tech Neck & Forward Head Posture

Melbourne's most common neck presentation by a significant margin. Hours at a laptop, looking down at a phone, working from a kitchen table that's never quite at the right height — it all creates the same pattern: a head that sits progressively further forward of the shoulders, placing enormous load on the posterior neck muscles and cervical spine. Every centimetre of forward head position adds roughly 4–5kg of effective load on the neck. We address both the immediate symptoms and the postural pattern driving them — including thoracic spine stiffness and shoulder blade control, which are almost always contributing.

Acute Neck Strain — "Woke Up and Can't Move"

You know this one. You went to bed fine and woke up unable to turn your head without significant pain. Often no clear cause — sometimes a sleeping position, sometimes a draught, sometimes your neck was just ready to complain. These acute presentations can be genuinely debilitating but typically respond very well to early treatment. Early gentle movement combined with targeted manual therapy almost always produces faster recovery than rest and waiting. Don't spend a week staring straight ahead — come in.

Cervicogenic Headaches

Headaches that originate from the cervical spine — typically felt at the base of the skull, radiating to the temples or behind the eyes — are one of the most under-recognised presentations we treat. They're often misdiagnosed as tension headaches or migraines and treated with medication that addresses the symptom rather than the source. The upper cervical joints (C1–C3) refer pain in predictable patterns that are very recognisable to a trained physio. Manual therapy directed at these joints is highly effective and evidence-based for cervicogenic headache, as supported by the Australian Physiotherapy Association.

Whiplash & Motor Vehicle Injuries

Even low-speed collisions cause significant cervical strain — the sudden deceleration forces muscles and ligaments well beyond their normal range in milliseconds. Whiplash injuries require specific management that respects the tissue healing timeline while encouraging early active movement. We're experienced in working with TAC claims and can coordinate with treating doctors and specialists when needed. Important: whiplash symptoms sometimes emerge days after an accident — don't dismiss neck pain that develops in the 48–72 hours following a collision.

Sports-Related Neck Pain

Cyclists in aggressive riding positions develop cervical stiffness and suboccipital tension from sustained neck extension. CrossFit athletes strain upper trapezius from overhead pressing and kipping movements. AFL players take direct contact to the neck and shoulder. Runners with upper cross syndrome develop neck tension from poor head carriage. Each requires a different approach — sport-specific assessment, position-specific treatment, and return-to-sport protocols that account for the actual demands of your activity.

Chronic Neck Pain & Central Sensitisation

When neck pain persists for months or years it becomes a fundamentally different problem. Central sensitisation — where the nervous system becomes hypersensitive to pain signals — often develops alongside structural issues, making treatment more complex. We address both the physical and neurological components, using pain education, graded movement exposure, and progressive loading alongside manual therapy. Research in the Journal of Manual & Manipulative Therapy consistently shows that combined manual therapy and exercise produces the best long-term outcomes for chronic neck pain.

For very minor, single-day stiffness — yes, often fine to wait 48 hours. But if neck pain has persisted for more than a few days, is progressively worsening, is accompanied by headaches, or is affecting your sleep and daily activities, waiting rarely helps. The Australian Physiotherapy Association research consistently shows that early intervention leads to faster recovery and reduces the risk of chronic pain developing. The longer mechanical neck pain is left untreated, the harder it becomes to resolve.

Usually a combination of sleeping position, pillow height, pre-existing muscle tension, and a neck that was already slightly irritated. The "I slept wrong" narrative is often not quite right — the position didn't cause the injury, it was just the thing that tipped an already-sensitive neck over the edge. Fixing it properly means treating the acute presentation and identifying why the neck was vulnerable in the first place. Pillow advice is part of most initial consultations for this presentation.

Yes — specifically for cervicogenic headaches (headaches that originate from the cervical spine, typically felt at the base of the skull or radiating to the temples). These are often misidentified as tension headaches or migraines. If your headaches are associated with neck stiffness, worsen when you turn your head, or consistently start at the back of the head, there's a good chance the cervical spine is involved. Manual therapy directed at the upper cervical joints is highly effective for this presentation and is one of the few truly evidence-based treatments for headache.

Acute neck strain: significant improvement typically within 1–3 sessions over 1–2 weeks. Cervicogenic headache: most patients see meaningful improvement within 4–6 sessions. Chronic neck pain: this varies more — improvement is usually noticeable within 4–6 weeks but full resolution may take 2–4 months depending on how long the problem has been present and how many contributing factors are involved. We'll give you a specific timeline at your first session based on your presentation.

Absolutely, and this is more physiologically real than most people realise. Stress activates the sympathetic nervous system, which increases muscle tone — particularly in the upper trapezius and suboccipital muscles. It also affects posture (hunched shoulders, forward head) and disrupts sleep, which reduces tissue recovery. Chronic neck pain and chronic stress also create a genuine feedback loop where each worsens the other. We address the physical drivers of this cycle, and for patients where psychological factors are significant, we can refer to allied mental health professionals.

Seek emergency care immediately if: neck pain follows significant trauma (car accident, fall from height), is accompanied by severe headache and fever, causes loss of consciousness, or produces weakness or numbness in both arms or legs. See a doctor or physio promptly if: you have numbness or tingling down one arm, severe pain that doesn't improve with rest, unexplained weight loss alongside the neck pain, or a history of cancer. For most other neck pain, coming to see us directly is the right first step — we'll refer you elsewhere if we identify anything that requires further investigation.

Very possibly. Common home office problems include laptops that force you to look downward, monitors at the wrong height, chairs with no neck or lumbar support, working from couches or beds, and the loss of incidental movement that naturally occurred in an office. We provide practical, specific ergonomic advice as part of most neck pain consultations — monitor height, chair position, keyboard placement, and movement break strategies. Small setup changes combined with targeted exercises produce dramatic improvements for WFH neck pain.
